Inspector observed peeled onions in container on floor of walk-in cooler. Corrective Action: food shall be protected from contamination by storing the food at least 6 inches (15 cm) above the floor.
12B - personal cleanliness
Regulation: 511-6-1.03(5)(j) - hair restraints (c)
Inspector observed employee handling food without hair restraint. COS- employee put on hair restraint. Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch.
18 - insects, rodents, and animals not present

Inspector observed that the walk-in cooler (with door that opens to exterior/outdoors) does not self close. CA: Outer openings of a food service establishment shall be protected against the entry of insects and rodents by solid, self-closing, tight-fitting doors.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Inspector observed dark-colored residues on floors underneath equipment, sinks, and non-food contact exteriors of food storage equipment in the main kitchen. CA: The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(2)(i) - light bulbs, protective shielding (c)
Inspector observed vent hood with unshielded, non-shatter resistant light bulbs. CA: light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food; clean equipment, utensils, and linens; or unwrapped single-service and single-use articles.
