12B - personal cleanliness
Regulation: 511-6-1.03(5)(j) - hair restraints (c)
Observed employees handling food without hair restraints. Corrective Action: Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch.
16B - plumbing installed; proper backflow devices
Regulation: 511-6-1.06(2)(r) - system maintained in good repair (p, c)
Inspector observed grease trap and septic lids not secured; lids missing screws. Corrective Action: A plumbing system shall be repaired according to law and maintained in good repair.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(d) - nonfood-contact surfaces (c)
Inspector observed interior of freezer and places between equipment in need of deep cleaning. Corrective Action: Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
17B - garbage/refuse properly disposed; facilities maintained
Regulation: 511-6-1.06(5)(r),(s) - frequency, removal of refuse, recyclables and returnables often enough to minimize odors and attracting pests; receptacles or vehicles, removal from premises in approved manner (c)
Inspector observed refuse around the back exterior of building. Corrective Action: Refuse, recyclables, and returnables shall be removed from the premises by way of:

Portable receptacles that are constructed and maintained according to law; or

A transport vehicle that is constructed, maintained, and operated according to law.
