Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch.
This does not apply to employees such as counter staff who only serve beverages and wrapped or packaged foods, hostesses, and wait staff if they present a minimal risk of contaminating exposed food, clean utensils and linens and unwrapped single-service and single-use articles.

(a) General Requirements. Utensils and food-contact surfaces of equipment shall be made of materials that do not allow the migration of deleterious substances or impart colors, odors, or tastes to food and under normal use conditions, and shall be: P safe P, durable, corrosion-resistant, and nonabsorbent. In addition, materials shall be sufficient in weight and thickness to withstand repeated warewashing and shall be finished to have a smooth, easily cleanable surface. Materials shall be resistant to pitting, chipping, crazing, scratching, scoring, distortion and decomposition. Note: Observed 2 large knifes with chipped blades above vegetable prep sink. Both ice cream machines/reach-in-coolers were above the required temperature of 41F or below, see temperature observations. PIC voluntarily discarded 5 bags of ice cream milk between the two machines to COS. Both machines/coolers are being put through a defrost cycle and then turned back on. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41F or below. Do not use ice cream machines/coolers until it can maintain a temperature of 41F or below. Have machines running and holding temperature of 41F or below (check by taking temperatures) prior to use. If defrosting the machines does not fix the temperature problem have them repaired and/or replaced.

Back door into facility is not tight-fitting, leaving a gap where you can see sunlight coming through. Outer openings of a food service establishment shall be protected against the entry of insects and rodents by solid, self-closing, tight-fitting doors. Repair and/or replace door such that it is tight-fitting and seals completely to prevent the entry of any insects or pests.
