Observed PIC inability to present current CFSM certificate at the time of inspection. (CA) Advised the PIC that The original CFSM certificate shall be posted in public view in each food service establishment. An additional copy shall be retained on file at the food service establishment at all times, and shall be made available for inspection by the Health Authority.

Observed the current permit has not been posted. (CA) Advised the PIC to pay for the permit and post the permit in public view. Post the permit in a location in the food service establishment that is conspicuous to consumers

Observed the current permit has not been paid so the most current permit cannot be posted. (CA) Advised the PIC to pay for the current permit and then post the permit. Post the permit in a location in the food service establishment that is conspicuous to consumers

Observed PIC inability to present or provide current CFSM in establishment. CA: Advised PIC Food service establishments shall have in its employ a Certified Food Safety Manager (CFSM) to ensure food safety is being managed within the food service establishment during all hours of operation.

Establishment failed to present previous inspection report & is currently behind on FSP annual fee. CA: Advised PIC to post most recent inspection report in an area visible for public view along with meeting requirements toward receiving current FSP to present for public view.

Observed cases of water stored on the floor in the main kitchen. CA: Advised pic that food shall be stored in a clean, dry location; where it is not exposed to splash, dust, or other contamination; and at least 6 inches (15 cm) above the floor. Food in packages and working containers may be stored less than 6 inches (15 cm) above the floor on case lot handling equipment if the equipment can be moved by hand or by conveniently available apparatuses such as hand trucks and forklifts.

The permit and inspection report is not posted in a location in the food service establishment that is conspicuous to the consumers. CA: advised PIC to post most recent permit and inspection report. The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away.
