Observed the floors and walls in the kitchen needed cleaning. (CA) Advised the PIC that the floors and walls needed to be cleaned more frequently. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.

511-6-1.02(1)(d) - Displaying of the Inspection Report (C)
The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away.
Food service establishments with drive-thru windows will post the current inspection report, and also have the inspection report posted so that a minimum of the top one-third of a copy of the current inspection report is visible through each window allowing customers to easily read the score, date of inspection and establishment information.
At food service establishments with no primary or public door, the current inspection report shall be prominently displayed at all times where the documents can be read by the public from a distance of one foot away. If requirements of this paragraph are not possible because of physical restrictions, a location will be determined as approved by the Health Authority.
The most current inspection report for mobile food service units and extended food service units issued by the local Health Authority having jurisdiction for its inspections shall be prominently displayed in public view during all hours of operation. Such inspection reports shall be prominently displayed at the point of service where the documents can be read by the public from a distance of one foot away.
A food service establishment inspection report addendum need not be displayed, but shall be made available by the food service establishment to the public upon request.

Observed pans of french fries being stored not covered on a sheet pan rack in the walk in cooler. Observed container of french fries being stored on the counter not covered. Observed containers of chicken wings being stored not covered in a reach in freezer. Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination. PIC provided lids for food items.

Observed rust and food buildup on the potato fry cutter. Equipment food-contact surfaces and utensils shall be cleaned: (i) Before each use with a different type of raw animal food such as beef, fish, lamb, pork, or poultry. It does not apply if the food-contact surface or utensil is in contact with a succession of different types of raw meat and raw poultry each requiring a higher cooking temperature as specified under DPH Rule 511-6-1.04(5)(a) than the previous type such as preparing raw pork followed by cutting raw poultry on the same cutting board; P (ii) Each time there is a change from working with raw foods to working with ready-to-eat foods; P (iii) Between uses with raw fruits and vegetables and with Time/Temperature Control for safety food; P (iv) Before using or storing a food temperature measuring device; P and (v) At any time during the operation when contamination may have occurred. P PIC had the fry cutter cleaned.

Observed an employee wearing three bracelets while preparing food. Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food. PIC had the employee remove the bracelets and wash hands.

Observed a broken container holding Parmesan cheese. Multiuse food-contact surfaces shall be: (i) Smooth; Pf (ii) Free of breaks, open seams, cracks, chips, inclusions, pits, and similar imperfections; Pf (iii) Free of sharp internal angles, corners, and crevices; Pf (iv) Finished to have smooth welds and joints; Pf and (v) Except as specified in paragraph 2 of this subsection, accessible for cleaning and inspection by one of the following methods: (I) Without being disassembled, Pf (II) By disassembling without the use of tools, Pf or (III) By easy disassembling with the use of handheld tools commonly available to maintenance and cleaning personnel such as screwdrivers, pliers, open-end wrenches, and Allen wrenches. Pf PIC had the Parmesan cheese moved to another container and the broken container discarded.

Observed heavy buildup of boneless chicken wings and ice on the interior walls and shelving of a reach in freezer near the fryers. Observed food and debris buildup in the prep top cooler and reach in cooler interior walls, gaskets and shelving. Observed buildup on the stainless steel walls of the fryers and range panels. Observed food buildup of the metal rack and food catch of a conveyer oven. 2. The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations. 3. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. PIC shall have non food contact surfaces cleaned.

Observed food and debris buildup on the floors of the walk in freezer and floor tiles of the main kitchen underneath the three compartment sink, hand sinks, prep sinks, and cooking equipment. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. PIC shall have the physical facilities cleaned.

Observed broken panels of FRP walls in the main kitchen underneath the wall mounted potato fry cutter. All physical facilities shall be maintained in good repair. PIC shall have the FRP wall repaired.

Observed a mop head drying directly in the mop sink. After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies. PIC had the mop head relocated to air dry.
