4-1A - food separated and protected
Regulation: 511-6-1.04(4)(c)1(i)(ii)(iii)(v)(vi)(vii)(viii) - packaged & unpackaged food separation, packaging, and segregation (p, c)
Observed raw chicken stored over raw beef in the main kitchen WIC and RIF near the grill area. CA: Advised PIC to change order as food shall be protected from cross contamination by:
(i) Except as specified in paragraph (i)(IV) of this subsection, separating raw animal foods during storage, preparation, holding, and display from:
(I) Raw ready-to-eat food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw ready-to-eat food such as fruits and vegetables, P and
(II) Cooked ready-to-eat food; and P
(III) Fruits and vegetables before they are washed; P
(IV) Frozen, commercially processed and packaged raw animal food may be stored or displayed with or above frozen, commercially processed and packaged, ready-to-eat food.
(ii) Except when combined as ingredients, separating types of raw animal foods from each other such as beef, fish, lamb, pork, and poultry during storage, preparation, holding, and display by:
(I) Using separate equipment for each type, P or
(II) Arranging each type of food in equipment so that cross contamination of one type with another is prevented, P and
(III) Preparing each type of food at different times or in separate areas; P
(iii) Cleaning and sanitizing equipment and utensils;
(v) Cleaning hermetically sealed containers of food of visible soil before opening;
(vi) Protecting food containers that are received packaged together in a case or overwrap from cuts when the case or overwrap is opened;
(vii) Storing damaged, spoiled, or recalled food being held in the food service establishment separate from food, equipment, utensils, linens and single-service and single-use articles; or
(viii) Separating fruits and vegetables, before they are washed from ready-to-eat food.

Observed black buildup in the interior of the main kitchen ice machine. CA: Advised PIC to routinely clean ice machine.
5-2 - consumer advisory provided for raw and undercooked foods
Regulation: 511-6-1-.04(7)(e) - consumer advisory provided for raw/undercooked
Observed no reminder or disclosure statement on the menu for sea bass ceviche. CA: Advised PIC to add reminder and disclosure statement on the menu for all items served raw or undercooked.
10D - food properly labeled; original container
Regulation: 511-6-1.04(7)(c) - food labels (pf, c)
Observed unlabeled dry goods outside of their original container in the main kitchen. Also, observed multiple sauce bottles outside of their original containers, such as tartar sauce and lemon juice, without labels. COS PIC labeled items.
11C - approved thawing methods used
Regulation: 511-6-1.04(6)(c) - thawing (c)
Observed fish sitting out on the counter. Also, observed seafood sitting in a sink filled with water. COS PIC placed all thawing items under running water that is drained.
11D - thermometers provided and accurate
Regulation: 511-6-1.05((2)(l) - tmd, ambient air & water - accuracy (pf)
Observed a lack of thermometers in the main kitchen RICs, WICs, and WIFs for accurately measuring ambient temperatures. CA: Advised PIC to obtain thermometers for ambient temperature measurements.
18 - insects, rodents, and animals not present
Regulation: 511-6-1.07(5)(k) - controlling pests (pf, c)
Observed multiple flies in the main kitchen. CA: Advised PIC to obtain pest control sufficient enough to control the presence of pests.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed water buildup on the floor of the outside WIC. Observed ice buildup on the floor of the main kitchen WIF. CA: Advised PIC to clean all nonfood contact surfaces.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed black build up on the floor in the bar area. Observed missing floor tiles near the WIF in the main kitchen. CA: Advised PIC to maintain all physical facilities in good repair.
