Observed the inability to produce the employee health agreement documents. CA: Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food
2-2E - response procedures for vomiting & diarrheal events
Regulation: 511-6-1.03(6) - clean-up of vomiting and diarrheal events(pf)
unable to produce a clean up kit and the written procedures. CA: PIC was advised to buy a clean up kit. (6) Responding To Contamination Events. A food establishment shall have written procedures for employees to follow and an EPA registered disinfectant effective against Norovirus available on-site at the food service establishment when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food service establishment. The procedures shall address the specific actions employees must take to minimize the spread of contamination and the exposure of employees, consumers, food, and surfaces to vomitus or fecal matter.Pf

Observed both ice machines with black slime on the white deflector plate. CA:Equipment, Food-Contact Surfaces, and Utensils.

Observed dishwashing machine at the bar with 0 chemical concentration. CA: PIC was advised to call ecolab to correct the issue. Note that the smell of rotten eggs (likely sulfur) should call a plumber to check on that

Observed the establishment failed to post their food permit, the establishment needs to obtain a valid food service permit. CA: PIC was advised to obtain a valid food service permit and legal notice was issued.
15B - warewashing facilities: installed, maintained, used; test strips
Regulation: 511-6-1.05(6)(p) - warewashing equipment, determining chemical sanitizer concentration (pf)
during the routine inspection establishment failed to provide testing strips for their dishwashing machine and the three compartment sink. CA: PIC was advised to obtain testing strips for three compartment sink (quat testing strips) and dishwasher (chlorine testing strips)
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
observed several holes in the wall of the bar area where water leaks out of it when someone in a near building flushes a or runs water. water from the walls leaks into the 3 compartment sink.
