Observed the facility not able to show in a verifiable manner that the employees are notified of the illnesses and symptoms they are required to report. Observed three skim milks dated January 19th. COS- milks were discarded. 511-6-1.04(8)(b) - Expired Foods (P) (b) Expired Foods. Prepackaged sandwiches, eggs, infant formula, shucked oysters, milk, and time/temperature control safety foods that are labeled as “keep refrigerated” and that are for sale or service to the consumer or used as an ingredient in other foods shall be immediately discarded and shall not be sold, served, or used after the manufacturer’s expiration date or the sell-by date. 12C - wiping cloths: properly used and stored

Observed an in use sanitizer bucket stored directly on the ground and the concentration below 200ppm quats. COS- bucket was remade to 200ppm quats and moved off the ground. 511-6-1.04(4)(m) - Wiping Cloths, Use Limitation (C) (m) Wiping Cloths, Use Limitation.
- Cloths in-use for wiping food spills from tableware and carry-out containers that occur as food is being served shall be:
(i) Maintained dry; and
(ii) Used for no other purpose.
- Cloths in-use for wiping counters and other equipment surfaces shall be:
(i) Held between uses in a chemical sanitizer solution at a concentration specified under DPH Rule 511-6-1-.05(6)(n); and 5. Containers of chemical sanitizing solutions in which wet wiping cloths are held between uses shall be stored off the floor and used in a manner that prevents contamination of food, equipment, utensils, linens, single-service, or single-use articles.
15B - warewashing facilities: installed, maintained, used; test strips

Observed a high temperature dish machine and no irreversible temperature indicator to measure the utensil surface temperature of the machine.
