28 - beds, mattresses, mattress pads, springs, frames, pillows, and bed coverings clean; vermin free and in good condition; bed linen properly supplied, changed and laundered

Observed stained bed mattress in room 605. Observed stained mattress pad room 242. Observed stained duvet covers in rooms 110 and 215. Observed stained pillows in room 405. Observed stained pillow cover in room 601. Instructed housekeeping lead to properly clean/sanitize following items during inspection. 511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly

Observed improper air ventilation circulation in guest bathrooms in rooms 414 and 215. Informed PIC to provide proper ventilation circulation for guest bathrooms during inspection. 511-6-2-.08(5) - Toilet and Bathing Ventilation Toilet, lavatory and bathing facilities shall be mechanically ventilated. Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. In existing tourist accommodations permitted prior to the adoption of this rule, the current ventilation must prevent odor, mold, mildew, and moisture. All new or major renovation of a tourist accommodation will require mechanical ventilation to be installed to the outside of the building separate from heating and air-condition systems

Observed no anti-slip surface materials provided in all tubs throughout hotel property. Instructed Housekeeping manager to provide anti slip surfaces in all rooms which holds tubs during inspection. 511-6-2-.08(10) - Anti-Slip Surface Anti-slip tubs slip strips, appliqués, or slip-proof mats shall be provided in each bathing facility and shall be kept clean and in good repair
