Observed multiple rooms with stains on bed mattresses . Advised PIC to replace linen and provide guest with clean linen free of holes and stains. 511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly

Observed wrong inspection report posted. Inspection shows a score of 100 when the last score was 91. 511-6-2-.05(7) - Inspection Posting The most recent tourist accommodation inspection report form shall be prominently displayed in public view at all times, between five feet and seven feet from the floor and in a public area such as the registration desk, where it can be read at a distance of one foot away

Observed Ice machines on 11th and 10th floors with build up of pink substance. Advised PIC that Ice machines need to be cleaned bi weekly. 511-6-2-.07(8) - Ice Supply If self-service ice is provided, then it shall be from an approved water supply. In all new tourist accommodations, and in existing tourist accommodations when machines are replaced, only automatic dispensing ice machines will be allowed. In existing tourist accommodations permitted prior to the adoption of this rule, the use of existing self-service ice-storage bins may be continued, provided that the machines are maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations. Further, a copy of the manufacturer’s cleaning instructions shall be maintained onsite for review at the request of the Health Authority. Scoops, ice buckets, and other ice handling equipment, shall be of easily cleanable material and construction. They shall be stored in a clean place and shall be kept clean. Glassware shall not be used to scoop ice

Observed dead water in the lavatory facilities of rooms 1449 and 1450. CA: Call water service professional to assist with plumbing issue. Contact health department when violation is remedied within 7 days
