Observed ventilation in multiple bathrooms not pulling air in. Advised PIC that ventilation needs to be pulling air from bathroom

Observed build up in ice machine on first floor. CA: advised PIC to have ice machine serviced and cleaned

Observed stained mattresses and bedding in rooms 102, 105, 108, 202, 204, 306, 308. CA: Advised PIC to replace those mattresses. Violation Code Reference 511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly

Observed no ventilation in room 108. CA: Advised PIC to have ventilation system serviced. 511-6-2-.08(5) - Toilet and Bathing Ventilation Toilet, lavatory and bathing facilities shall be mechanically ventilated. Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. In existing tourist accommodations permitted prior to the adoption of this rule, the current ventilation must prevent odor, mold, mildew, and moisture. All new or major renovation of a tourist accommodation will require mechanical ventilation to be installed to the outside of the building separate from heating and air-condition systems

Observed pink build up in ice machine on the third floor. CA: Advised PIC to dump ice and sanitize ice machine. 511-6-2-.07(8) - Ice Supply If self-service ice is provided, then it shall be from an approved water supply. In all new tourist accommodations, and in existing tourist accommodations when machines are replaced, only automatic dispensing ice machines will be allowed. In existing tourist accommodations permitted prior to the adoption of this rule, the use of existing self-service ice-storage bins may be continued, provided that the machines are maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations. Further, a copy of the manufacturer’s cleaning instructions shall be maintained onsite for review at the request of the Health Authority. Scoops, ice buckets, and other ice handling equipment, shall be of easily cleanable material and construction. They shall be stored in a clean place and shall be kept clean. Glassware shall not be used to scoop ice

Observed no anti-slip mat or coating in tubs in rooms 102, 105, 108, 202, 204, 306, 308. CA: Advised PIC to place anti-slip mat or coating in all tubs and showers. 511-6-2-.08(10) - Anti-Slip Surface Anti-slip tubs slip strips, appliqués, or slip-proof mats shall be provided in each bathing facility and shall be kept clean and in good repair

Observe several unmarked chemical spray bottles on the house keeping carts. CA: Advised PIC to label items with common name. 511-6-2-.17(2) - Housekeeping Carts Housekeeper carts shall be so arranged that clean replacement supplies, clean linens, and cleaned and sanitized multi-use equipment and utensils shall be protected from soiled items being removed from each room. Soiled linens and refuse shall be placed in appropriate containers on housekeepers’ carts. Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents. Cleaners and sanitizer shall be used according to the manufacturer’s use direction and label

Observed mattresses with heavy stains in rooms 125, 131, 227 105 and stained pillows in rooms 105. C/A: Advised PIC to change bedding’s when cleaning of stains become unattainable

Observed an INJURY HAZARD from broken glass at FDC in stairwell to third floor entry. C/A: Advised PIC to repair broken glass to prevent injury and maintain in good repair

Observed the build up of mold like accumulation on shower heads in rooms 105, 107, and 337 and where applicable. C/A: PIC shall clean shower heads at a frequency that precludes accumulation of soil and mold like substance

Observed a failure to maintain ventilation vents in bathrooms of rooms 235. 227, 107 and where applicable in good repair. C/A: PIC must ensure vents are operable to prevent the accumulation of moisture, odors, mildew or build up of mold

Observed a failure to keep ice machine chutes in a clean and sanitary manner. POC: Advised PIC to clean ice machine at frequency and specifications of the manufacturer and develop a cleaning log to be maintained on site and made available for review by EHS staff
