Observed failure to maintain outer openings protected Observed back door of kitchen not properly sealed around causing light eminating from Sind and bottom. Except as specified in paragraphs 2 through 5 of this subsection, outer openings of a food service establishment shall be protected against the entry of insects and rodents by:
(i) Filling or closing holes and other gaps along floors, walls, and ceilings;
(ii) Closed, tight-fitting windows; and
(iii) Solid, self-closing, tight-fitting doors. PIC shall reseal door

The delivery tube or chute and orifice of equipment used to vend liquid food or ice in unpackaged form to self-service consumers shall be designed so that the delivery tube or chute and orifice are protected from dust, insects, rodents, and other contamination by a self-closing door if the equipment is PIC removed from floor

Observed TCS foods (black beans, steak, and ground beef) being Hot-held at a temperature below 135 degrees. COS: PIC began the process of reheating tcs food items at a temperature of 165 degrees.
