CA: Facility not in operations at the time of my visit due to death in the family. However, owner was informed not to operation facility until the hot water is restored at base.

CA: Replace ceiling tiles at the base of operation with an approvable tile. Tiles need to be nonabsorbent, smooth and easily cleanable.
