Observed ceiling tiles unclean and not in good repair in the main kitchen. CA: Advised PIC physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products and maintained in good repair.

Observed a pink mold like substance on the baffle of the bulk ice machine. CA: Advised PIC surfaces of utensils and equipment contacting food that is not time/temperature control for safety food shall be cleaned: (i) At any time when contamination may have occurred; In equipment such as ice bins and beverage dispensing nozzles and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment.

Observed Water leaking from ceiling in storage room. Advised PIC to have necessary repairs to facility. (r) System Maintained in Good Repair. A plumbing system shall be repaired according to law; P and maintained in good repair.

Observed cardboard boxes of food being stored on the floor in a hallway. Except as specified in paragraphs 2 and 3 of this subsection, food shall be protected from contamination by storing the food: (i) In a clean, dry location; (ii) Where it is not exposed to splash, dust, or other contamination; and (iii) At least 6 inches (15 cm) above the floor. PIC had the boxes discarded.

Observed cracked and broken floor tiles in the dish pit, office, and mop sink. Good Repair. All physical facilities shall be maintained in good repair. Observed discoloration on the drop ceiling tiles. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. PIC shall have the floor tiles repaired. PIC shall have the drop ceiling tiles cleaned.

Except when dry cleaning methods are used as specified under subsection (7)(e) of this Rule, surfaces of utensils and equipment contacting food that is not time/temperature control for safety food shall be cleaned: (i) At any time when contamination may have occurred; In equipment such as ice bins and beverage dispensing nozzles and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment. PIC had ice burned and the bulk ice machine cleaned.
