Observed that the PIC could not provide documentation or otherwise satisfactorily demonstrate during the inspection, that all food employees and conditional employees are informed of their responsibility to report to management information about their health and activities as it relates to diseases that are transmissible through food. 511-6-1.03(4)(a)(b)(c)(e)(f) - Responsibility of permit holder, PIC, and Conditional employees requirement to Report Symptoms, Diagnosis and History of Exposure. The permit holder shall require food employees and conditional employees to report to the CFSM and person in charge, information about their health and activities as they relate to diseases that are transmissible through food. A copy was sent to PIC again.

Observed black mold-like marks on the surface of the cutting board at the front food service area. Food contact surfaces shall be cleaned and sanitized in between uses. Advised PIC to sanitize the cutting boards.

Observed failure to set up sanitizer bucket to clean food prep tables.

Observed failure to provide hot water at the hand sink at the front food service area.
