2-2A - management knowledge, responsibilities, reporting

Observed failure to provide employee agreements to let the PIC know their responsibilities when feeling sick or having a sickness. (a) Requirement to Report Symptoms, Diagnosis and History of Exposure. The permit holder shall require food employees and conditional employees to report to the CFSM and person in charge, information about their health and activities as they relate to diseases that are transmissible through food. A food employee or conditional employee shall report the information in a manner that allows the CFSM and person in charge to reduce the risk of foodborne disease transmission, including providing necessary additional information, such as the date of onset of symptoms and an illness, or of a diagnosis without symptoms, if the food employee or conditional employee: PIC advised to get employee health forms and get them signed.
18 - insects, rodents, and animals not present

Observed failure to control pests inside the facility. Observed multiple fruit flies and other insects around the food in front food prep area and main kitchen. (k) Controlling Pests. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by:

Routinely inspecting incoming shipments of food and supplies;

Routinely inspecting the premises for evidence of pests;

Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule; Pf and PIC advised to have pest control services provide fly traps.

Observed failure to post the most recent inspection report in customer view inside facility and on drive thru window.

The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away.

Food service establishments with drive-thru windows will post the current inspection report, and also have the inspection report posted so that a minimum of the top one-third of a copy of the current inspection report is visible through each window allowing customers to easily read the score, date of inspection and establishment information. PIC advised to post inspection report within ten days.
14A - in-use utensils: properly stored

Observed failure to properly store handled utensils. Observed multiple utensils in bins of protein powder and cinnamon with handles touching food. (k)Storage of In-Use Utensils. During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored:
