Observed pink and black mold substance in ice machine interior white flap. 511-6-1.05(7)(b) - Food Contact Surfaces and Utensils - Cleaning Frequency (P, C) (b) Equipment Food-Contact Surfaces and Utensils. PIC will clean and maintain clean.
6-2 - proper date marking and disposition
Regulation: 511-6-1.04(6)(h) - ready-to-eat time/temperature control for safety food, disposition (p)
Observed failure to properly date mark potentially hazardous food prepared on sit and held for more than 24 hours inside walk in cooler (rice, potatoes, chicken and etc). 511-6-1.04(6)(h) - Ready-To-Eat Time/Temperature Control for Safety Food, Disposition (P) (h) Ready-to-Eat, Time/Temperature Control for Safety Food, Disposition.
- A food that requires datemarking shall be discarded if it:
(i) Exceeds 7 days, not including the time that the product is frozen; P
(ii) Is in a container or package that does not bear a date or day; P or
(iii) Is inappropriately marked with a date or day that exceeds 7 days. PIC instructed that all ready-to-eat, potentially hazardous food held more than 24 hours must have a prepared or discard date.

- Except as specified in paragraph 4 of this subsection, cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored:
(i) In a clean, dry location;
(ii) Where they are not exposed to splash, dust, or other contamination; and
(iii) At least 6 inches (15 centimeters) above the floor. PIC moved boxes off the floor.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed broken tile underneath the hand sink. observed failure to provide smooth, nonporous, and easily cleanable ceiling tiles in the dry storage room across from the kitchen and above the reach-in freezer in the main kitchen (2 ceiling tiles). PIC was advised that the ceiling tiles must be smooth, durable, and easily cleanable.
