6 - premises free of pests and rodents; pesticide applied in accordance with laws, rules, and label
Regulation: 511-6-2-.13(1) - insect and rodent control
Observed gnat infestation in one room and water bugs in another room. 511-6-2-.13(1) - Insect and Rodent Control Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents. Restricted use pesticides, as specified in 40 CFR 152 Subpart I - Classification of Pesticides shall be applied only by a licensed professional. All pesticide applications must be in accordance with current state and federal laws and the product label. Applying pesticides within buildings using area fog dispersal methods or as warranted under DPH Rule-.20(4)(c) is restricted to licensed pest control professionals as part of an integrated pest management program
8 - annual gas fired equipment inspection on file; gas units equipped with automatic safety and/or 100% cut-off pilot
PIC could not provide the annual gas fired equipment form provided on the last inspection 511-6-2-.15(5) - Heating and Fire Safety-Inspection. All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency
28 - beds, mattresses, mattress pads, springs, frames, pillows, and bed coverings clean; vermin free and in good condition; bed linen properly supplied, changed and laundered
Observed beds, linens and mattresses with stains in all the rooms inspected. 511-6-2-.14(6) - Bed, Bedding, and Linens. Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly. Stained mattresses, linen and pillows shall be replaced or cleaned as such to remove stains; if stains cannot be removed, then it must be replaced. CA: Advised PIC to clean, or replace all bed linen, mattress, pillows and covering that has stains and tears

Observed multiple ventilation fans in the bathrooms out of order, missing, or in need of new motors. Advised PIC that these need maintenance

Observed stains on walls, floors ceilings and carpets through out facility. Ceiling in hallway with peeling popcorn; peeling paint on wall’s through out facility. 511-6-2-.14(1) - Floors, Walls, and Ceilings Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. In all new constructed and extensively remodeled residential kitchens used in bed and breakfast inns, coved base molding may be required. Walls and ceilings in residential kitchens shall be easily cleanable and light colored.
