Observed no glove change and hand-washing in-between handling non-ready to eat food such as handling raw beef to cook a burger to ready to eat food (the lettuce, tomato, and cheese) to prevent contamination. Discussed with PIC and cook to follow proper hand washing and glove exchange procedures to prevent cross-contamination. PIC instructed cook to follow proper glove exchange procedures.

Observed adult and juvenile german roaches on walls and floors, in kitchen area and dishpit. Use commercial grade chemicals or services for pest control

Observed raw burger patties stored over cooked prime rib. Raw foods must be stored below ready to eat foods. COS employee rearranged foods to comply with code.

Observed multiple TCS foods held past appropriate discard date. TCS foods must be discarded within 7 days of make/open date, including the day the product was opened/made. COS employee discarded foods that were out of date.

Observed no available test strips for testing the sanitizer at the 3 compartment sink. Provide test strips for 3 compartment sink.

Observed sanitizer not dispensing in warewashing machine. Chlorine sanitizer must be present 50-100ppm. COS- Employees will use 3 comp sink until warewashing machine dispenses chlorine correctly.
