Observed no hadnwashing soap at handwashing sink. CA: Each handwashing sink shall be provided with a supply of hand cleaning liquid, powder, or bar soap. COS: PIC provided soap at handwashing sink.

Observed no paper towels at handwashing sink. CA: Each handwashing sink shall be provided with individual, disposable towels. COS: PIC provided paper towels to handwashing sink.
2-2E - response procedures for vomiting & diarrheal events
Regulation: 511-6-1.03(6) - clean-up of vomiting and diarrheal events(pf)
Observed no bodily fluid clean-up kit or written procedures at facility. CA: A food establishment shall have written procedures for employees to follow and an EPA registered disinfectant effective against Norovirus available on-site at the food service establishment when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food service establishment. COS: PIC ordered bodily fluid clean-up kit. Will perform informal follow-up on 5/15 to verify.

Observed facility without consumer notification for the presence of the 9 major food allergens posted for public view. CA: The permit holder shall notify consumers by written notification of the presence of major food allergens as an ingredient in unpackaged food items that are served or sold to the consumer. COS: PIC displayed written allergen poster at front counter for public view.
10D - food properly labeled; original container
Regulation: 511-6-1.04(8)(b) - expired foods (p)
Observed raw shelled eggs with manufacturers use but date of 5/5 and inspection performed on 5/12. CA: Eggs and time/temperature control safety foods that are labeled as “keep refrigerated” and that are for sale or service to the consumer or used as an ingredient in other foods shall be immediately discarded and shall not be sold, served, or used after the manufacturer’s expiration date or the sell-by date. COS: PIC discarded eggs.
17A - toilet facilities: properly constructed, supplied, cleaned
Regulation: 511-6-1.07(2)(l) - toilet rooms, enclosed (c)
Observed restroom self closure in disrepair. CA: Restrooms must be provided with a tight-fitting and self-closing door. No COS can be observed. PIC must repair self closure by 5/15.
