Observed bleach spray bottle stored next to sauce to go lids. CA: Toxic materials shall be separated from food and equipment. COS: CFSM relocated chemical spray bottle to area separate from food, equipment, and utensils.

Observed boxes of food product in walk in cooler stored directly on floor. CA: All food shall be stored at least 6 inches from the floor. COS: CFSM relocated boxes to shelving.

Regulation: 511-6-1.05(10)(e)1,2,4 - equipment, utensil, linens, stored 6" off floor in clean, dry location (c)
Observed boxes of trays stored directly on floor in main kitchen. CA: All equipment and utensils shall be stored at least 6 inches from the floor. COS: CFSM relocated boxes of trays to shelving off the floor.

Regulation: 511-6-1.05(1)(a) - materials, general requirements (p, c)
Observed cardboard box being used to hold chicken breading and wash. CA: All food and non-food contact surfaces shall be cleanable and made of non-absorbent materials. COS: CFSM discarded cardboard box and will utilize a cleanable and non-absorbent material for food and non-food contact surfaces.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed damaged tiles and missing drain covers in main kitchen. CA: All physical facilities shall be in good repair. COS: Not noted as corrected on site. Floors will need to be repaired and rechecked by Health Department.
