NO CURRENT CFSM IN THE FACILITY. SHIFT MANAGER CONTACTED THE LEAD MANAGER - CHASITY GOSA - WHO STATED SHE DID HAVE A COPY OF HER CFSM BUT COULD NOT LOCATE IT. SHE WAS GIVEN ROUGHLY 20 MINUTES TO LOCATE AND SEND TO THE FACILITY.

OBSERVED PIZZA CUTTING KNIFE BEING STORED WITH OLD FOOD CRUSTED ON. NO PIZZAS WERE COMING THROUGH THE OVEN LINE. PIC REPLACED THE DIRTY KNIFE WITH A CLEAN ONE.

OBSERVED SEVERAL BROOMS STORED WITHIN THE FACILITY NOT BEING USED. BROOMS ARE TO BE STORED ON HANGERS WHERE THE MOP SINK IS LOCATED.
