NOTE: All cold held temperatures were in compliance unless otherwise noted. NOTE: Facility uses frozen spinach that does have cooking instructions and states to heat spinach to 165F prior to use. If facility wants to continue to use the spinach, must apply for a remodel of facility to be able to cook spinach on-site or switch to another frozen spinach that does not have cooking instructions on package. 2-2A: .03(2)(o)Both employees on site did not have a verifiable way to ensure that employees are aware of health policy. Food employees and conditional employees shall be informed in a verifiable manner of their responsibility to report to the person in charge information about their health and activities as they relate to diseases that are transmissible through food. (Pf) Corrective Actions: PIC (person in charge) had herself and employee sign health forms.
