Have all violations corrected by August 14, 2025 or permit may be suspended. NOTE: The dishwasher is out of detergent but is sanitizing at high temp. The facility is currently washing by hand, rinsing, and then sanitizing with the dishwasher. NOTE: Observed ice starting to collect on the hoses in the top of the walk-in freezer. Ensure that the ice does not accumulate on the condenser or the boxes below.
1-2A: .03(2)(a)-(l), (n) Observed that the facility has a Key drop agreement. The invoices do not show temperatures being recorded per the shown agreement.// The food service establishment shall maintain records of the written agreement as specified in subsection (3)(m)3 of this Rule as well as records that show the delivery condition and temperature of the products upon receipt of delivery. Records shall be made available upon request by the Health Authority. (Pf)
10D: .04(4)(d)Observed working containers of peanuts, red pepper, garlic, candied walnuts, sweet and sour sauce, and cooking oil that were not labeled with the common name of the food item.// Except for containers holding food that can be readily and unmistakably recognized, such as dry pasta, working containers holding food or food ingredients that are removed from their original packages for use in the food establishment, such as cooking oils, flour, herbs, potato flakes, salt, spices, and sugar shall be clearly and legibly identified, in English, with the common name of the food. (C) Corrective Actions: Person in charge labeled the working containers with the common food name.//
14C: .05(10)(e)1&3Observed single use lids and cups stored under the drink machine in the drive thru. The drink machine has leaked and the boxes to the single use items were wet.// Single-service and single-use articles may not be stored in or under any source of contamination, including in locker rooms, in toilet rooms, in garbage rooms, in mechanical rooms, under sewer lines, under leaking water lines including leaking automatic fire sprinkler heads or under lines on which water has condensed; under open stairwells; or under other sources of contamination. (C) Corrective Actions: Person in charge removed the single use items from underneath the drink machine. The single use items were placed in a dry location.//
