All cold holding and hot holding temperatures in compliance unless otherwise noted. NOTE: An informal inspection will occur on April 16, 2026. Have all violations corrected by April 16, 2026 or permit may be suspended. NOTE: Ensure that ALL employees that have a beard longer than 1/2" and are working with or preparing food must wear a beard net. NOTE: Ensure that ALL employee drinks are in a disposable cup with a lid and straw when not stored in the office. NOTE: New Rules Checklist was reviewed with the Person in charge. 6-1A: .04(6)(f)Observed cut lettuce, cut tomatoes, asian salad, cheese slices, and cole slaw not being held at 41° F or below on the center prep line. The cooler was found to be in temperature. ( See Temp Log).// Time/temperature control for safety food shall be maintained at 41°F (5°C) or below. (P) Corrective Actions: The cut lettuce, asian salad, cheese slices and cole slaw were discarded.//

8-2B: .07(6)(c)Observed that a bottle of Purell sanitizer spray was stored next to a tray of uncovered cookies on the prep counter on the front service line.// Poisonous or toxic materials shall be stored so they cannot contaminate food, equipment, utensils, linens, and single-service and single-use articles by:
- Separating the poisonous or toxic materials by spacing or partitioning; (P) and
- Locating the poisonous or toxic materials in an area that is not above food, equipment, utensils, linens, and single-service or single-use articles. (P) Corrective Actions: The person in charge removed the sanitizer spray to a designated space for chemicals and chose to discard the cookies.//

15B: .05(3)(h),(i)Observed that the facility uses Purell sanitizer spray for the tables. The facility does not have test strips for the Purell Sanitizer spray to ensure that the proper concentration is maintained.// A test kit or other device that accurately measures the concentration in mg/L (or ppm) of sanitizing solutions shall be provided. 18: .07(2)(m)Observed that the back door has a gap at the threshold where sunlight is visible when completely closed.// Outer openings of a food service establishment shall be protected against the entry of insects and rodents by:
- filling or closing holes and other gaps along floors, walls, and ceilings; (C)
- closed, tight-fitting windows; (C) and
- solid self-closing, tight-fitting doors. (C)
