Food employees and conditional employees shall be informed in a verifiable manner of their responsibility to report to the person in charge information about their health and activities as they relate to diseases that are transmissible through food. (Pf) Corrective Actions: Both employees onsite signed employee health agreements.

2-2D: .06(2)(o)Observed hand sink by drink prep area in kitchen being used to dump drinks and ice. SECOND CONSECUTIVE VIOLATION A handwashing sink shall be maintained so that it is accessible at all times for employee use and not used for purposes other than handwashing. An automatic handwashing facility shall be used in accordance with manufacturer's instructions. (Pf) Corrective Actions: Hand sink was cleared.

4-2B: .05(6)(n)Observed in-use dish machines in both main kitchen and memory care areas dispensing sanitizer at too low of a concentration of chlorine. A chemical sanitizer used in a sanitizing solution for a manual or mechanical operations must meet a specified minimum concentration (for chlorine sanitizer it must be 50-100 ppm; for quat ammonia, it must be used according to the manufacturer's specifications). (P) Corrective Actions: Dish machines were primed.

8-2B: .07(6)(g)Observed sanitizer in wiping cloth bucket and in three-compartment sink at too high of a concentration of chlorine. SECOND CONSECUTIVE VIOLATION Chemical sanitizers, including chemical sanitizing solutions generated on-site, and other chemical antimicrobials used on food contact surfaces shall not exceed stated concentrations as listed on the EPA-registered label, in 40 CFR 180.940 and 40 CFR 180.2020, (100 ppm for chlorine and 400 ppm for quaternary ammonia). (P) Corrective Actions: Sanitizer was remade.

10D: .04(4)(d)Observed multiple squeeze bottles of oil, water, and liquid butter without labels. Except for containers holding food that can be readily and unmistakably recognized, such as dry pasta, working containers holding food or food ingredients that are removed from their original packages for use in the food establishment, such as cooking oils, flour, herbs, potato flakes, salt, spices, and sugar shall be clearly and legibly identified, in English, with the common name of the food. (C) Corrective Actions: Items were labeled.
