NOTE: All cold holding temperatures assessed in compliance unless otherwise noted. Food in freezer frozen solid. Ensure items are not stored in any handwashing sink. An informal inspection will be conducted by 5/19/2025. Ensure violation is corrected or permit may be suspended. Floors in kitchen and bar area must be regrouted to prevent standing water. 3-1D: .04(5)(f)Facility does not have a parasite destruction letter for the salmon from the supplier they buy from. // If raw or partially cooked fish are served or sold in ready-to-eat form, and the fish are aquacultured, letter of aquaculture from the supplier or aquaculturist stipulating that the fish were raised and fed according to subsection (5)(e)2(iv) this Rule shall be obtained and maintained in the food service establishment for 90 calendar days beyond the time of service or sale of the fish. 4-2B: .05(6)(n)Observed multiple items (container lid, blender blade, metal cups, strainers) being sanitized at the 3-compartment sink located at the bar, in Quat sanitizer with a concentration below the manufacturer's specifications. // A chemical sanitizer used in a sanitizing solution for a manual or mechanical operations must meet a specified minimum concentration (for chlorine sanitizer it must be 50-100 ppm; for quat ammonia, it must be used according to the manufacturer's specifications). (P) Corrective Actions: Sanitizer was remade following the manufacturer's specifications (150ppm-400ppm). //

Observed butane lighters stored above coffee containers. // Poisonous or toxic materials shall be stored and displayed for retail sale so they cannot contaminate food, equipment, utensils, linens, and single-service and single-use articles. (P) Corrective Actions: Chemical bottle and lighters were moved so they cannot contaminate items. //
