The permit was suspended and subsequently reinstated following a third repeat violation of .04(6)(h). Required training was completed with employees. 2-2A: .03(2)(o)Employee Health Agreement form was not provided for the selected employee. Food employees and conditional employees shall be informed in a verifiable manner of their responsibility to report to the person in charge information about their health and activities as they relate to diseases that are transmissible through food. (Pf) Corrective Actions: The requirements were discussed with the employee.

6-1A: .04(6)(f)Time/temperature control for safety (TCS) foods were observed held above 41°F in the reach-in cooler across from the grill and in the indoor walk-in cooler. Time/temperature control for safety food shall be maintained at 41°F (5°C) or below. (P) Corrective Actions: The foods have been holding above 41°F for an unknown amount of time.

6-2: .04(6)(h)Two containers of cooked intestines labeled with an expiration date of 6/6/26 at 4:00 PM, one container of cooked chicharrón labeled with an expiration date of 6/6/26 at 4:00 PM, one container of cooked tongue labeled with an expiration date of 5/16/26, and one container of cooked pork labeled with an expiration date of 5/23/26 were observed in the cooler. A food that requires datemarking shall be discarded if it exceeds 7 days, not including the time that the product is frozen. (P) Corrective Actions: The foods were discarded.

15A: .05(6)(q)1&3The ambient air temperature of the preparation cooler across from the grill is 49°F. Utensils shall be maintained in a state of repair or condition that complies with the requirements specified under subsections .05(1) and .05(2) or shall be discarded. (C) Ambient air temperature, water pressure, and water temperature measuring devices shall be maintained in good repair and be accurate within the intended range of use.

15B: .05(3)(h),(i)Chlorine is used as the food-contact surface sanitizer, and the available chlorine test strips were found to be expired (expiration year 2025). A test kit or other device that accurately measures the concentration in mg/L (or ppm) of sanitizing solutions shall be provided. (Pf)
