Note: Facility uses flavor syrups, vanilla extract as an additive(s). Note: Facility is not actively participating in key drops ar this time. Note: An informal inspection will be done by 06/05/2024 tto ensure violations are corrected. Note: Ensure to have designated area with a sign for the dump sink and hand washing sink. 12B: .03(5)(g)Observed employee wearing braclets while handing food. // Food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food (except for a plain ring such as a wedding band). (C) Corrective Actions: Employee removed braclets. //

12B: .03(5)(i)Observed two employee with beards longer than 0.5 with out a proper hair restraint. // Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch (1/2"). (C) Corrective Actions: Employees donned beard restraints. //

14A: .04(4)(k) During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored: 1. in the food with their handles above the top of the food and the container; 2. in running water of sufficient velocity to flush particulates to the drain, if used with moist food such as ice cream or mashed potatoes; 3. in a clean, dry container; or 4. in a container of water if the water is maintained at a temperature of at least 135°F (57°C) and the container is cleaned at the required frequency. 14B: .05(10)(e) 1,2,4Observed clean containers, bowls that were washed this morning and the night before in the ware wash station, in the shelf with clean containers. // Clean equipment and utensils shall be stored in a self-draining position that allows air drying and covered or inverted. (C) Corrective Actions: Containers were rewashed and placed in a self draining positon. //

17B: .06(5)(r),(s)Observed grease leaking from grease trap into the road, parking lot. // Soiled receptacles and waste handling units for refuse, recyclables, and returnables shall be cleaned at a frequency necessary to prevent them from developing a buildup of soil or becoming attractants for insects and rodents. (C)

17D: .07(2)(i)Observed light bulbs in the front of prep line that are not shatterproof. // Shielded, coated, or otherwise shatter-resistant bulbs need not be used in areas used only for storing food in unopened packages, if: (i) The integrity of the packages cannot be affected by broken glass falling onto them; and (ii) The packages are capable of being cleaned of debris from broken bulbs before the packages are opened. (C)
