2-2A: .03(1)(c)2,3,17Person in charge was not aware of the symptoms and diseases that employees have to report to them or of the diseases and symptoms that have to be reported to the health department. Person in charge shall explain their responsibility for preventing the transmission of foodborne disease by a food employee who has a disease or medical condition that may cause foodborne disease and can describe the symptoms associated with the diseases that are transmissible through food. They also shall be able to explain how food employees and conditional employees comply with reporting responsibilities and exclusion or restriction of food employees. (Pf) Corrective Actions: Went over the symptoms and diseases that have to be reported as well as making sure there is signage.

4-2B: .05(7)(a)1Observed mold-like substance inside ice machine. Equipment food-contact surfaces and utensils shall be clean to sight and touch. (Pf) Corrective Actions: Ice machine cleaned and sanitized.

5-1B: .04(5)(h)Observed kimchi soup being reheat in microwave for hot hold to a temp below 165 degrees F. Time/temperature control for safety food reheated in a microwave oven for hot holding shall be reheated so that all parts of the food reach a temperature of at least 165°F (74°C) and the food is rotated or stirred, covered, and then allowed to stand covered for 2 minutes after reheating. (P) Corrective Actions: Added more time to the soup so that it could be reheated properly.
