Observed no hot water available at restroom/shower located near lakeview sites (tent) #49 & #51. Observed hot water to reach peak temperatures of 123°F (men's), 124°F (men's), 124°F (women's) at restroom/shower located near lakefront sites (tent) #27 & #28. Observed hot water to reach peak temperatures of 124°F (men's), 128°F (men's), 127°F (women's), 129°F (women's) at restroom/shower located near lakefront sites (tent) #87 & #88. Cold running water under pressure shall be provided to all equipment that uses water. Hot and cold running water under pressure shall be provided to all lavatories, bathing facilities, laundry facilities, and water-using equipment where eating and drinking utensils are washed. In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120F. Corrective Actions: PIC stated restroom/showers without hot water will be closed until hot water access is restored. All other restroom/showers will be adjusted to lower hot water temperatures below 120°F

Violation of Code: [511-6-2-.09(6)] Observed water supply tower and hose at dump station not colored red. Each dump station shall have an available water supply for the flushing of dump station areas and the following shall apply: (c) The washing water supply towers, connections, hoses and other parts shall be colored red

Observed various types of insects (including flying insects that collected around the sinks and spiders) throughout all restrooms/showers. Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents. Restricted use pesticides, as specified in 40 CFR 152 Subpart I - Classification of Pesticides shall be applied only by a licensed professional. All pesticide applications must be in accordance with current state and federal laws and the product label. Applying pesticides within buildings using area fog dispersal methods or as warranted under DPH Rule-.20(4)(c) is restricted to licensed pest control professionals as part of an integrated pest management program

The damaged lid was observed on top of the Dumpster

The following restroom hand sinks were above 120F: Restroom S00705: men's: 133F, women's: 125F, Restroom S00704: men's: 131F, women's 131F, Restroom S00703: men's: 142F, women's: 134F, Restroom S00706: men's: 144F, women's: 142F, Restroom S00712: men's: 120F, women's: 126F. In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120F. The following clean out caps were found to be in disrepair: Restroom S00705, S00703, and S00712. The dump station riser was found to be in disrepair.

Such sewage disposal systems shall be constructed and maintained in a manner to prevent the creation of unsanitary conditions.

Note: EHS educated PIC on proper maintenance of septic systems

Observed there to be mold-like substances in showers of restrooms: S00705, S00704, S00703,S00706, and S00712.

Current ventilation must prevent odor, mold, mildew, and moisture.

Note: PIC will call appropriate personnel to have showers properly cleaned and maintained

Observed white hose connected to the dump station flush tower hose.
