8 - annual gas fired equipment inspection on file; gas units equipped with automatic safety and/or 100% cut-off pilot
Regulation: 511-6-2-.15(5) - heating and fire safety-inspection
First Consecutive Repeat Violation PIC was unable to provide gas-fired equipment inspection reports for water heaters and dryers. All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency. Corrective Actions: PIC to call licensed contractor or local fire authority to inspect gas-fired equipment. EHS provided blank copy of DPH Gas-Fired Equipment Inspection Report for reference
27 - all furniture, waste receptacles, draperies, carpets; clean, and in good repair, considered the property of and furnished by the tourist accommodation
Regulation: 511-6-2-.14(5) - condition of furnishings
First Consecutive Repeat Violations Observed smoke detectors in all rooms inspected to be in disrepair or missing batteries. Observed interior surfaces of microwaves in rooms #113 and #212 to have paint chipping and what appeared to be rust accumulation. Observed accumulation of food debris in gaskets on fridges in rooms #113, #208, #212, #219. Observed rust accumulation on top of fridge in room #113. All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin. Corrective Actions: PIC to ensure housekeeping informs manager of smoke detector issues and ensure cleanliness of microwaves. PIC to ensure housekeeping staff clean gaskets on fridges and replace fridges and microwaves as needed
28 - beds, mattresses, mattress pads, springs, frames, pillows, and bed coverings clean; vermin free and in good condition; bed linen properly supplied, changed and laundered
Regulation: 511-6-2-.14(6) - bed, bedding, and linens
Third Consecutive Repeat Violation Observed staining/holes on pillow covers, mattress, and bed coverings in rooms #113 and #219. Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly. Corrective Actions: PIC to clean and replace pillow covers, mattresses, and bed coverings
16 - toilet/lavatory/bathing facility fixtures and rooms clean and maintained; properly lighted, clearly marked, proper signage, and all in good repair
Regulation: 511-6-2-.08(4) - toilet facilities-cleaning and maintenance
Observed accumulation of orange substance on showerhead nozzles in rooms #113, #208, and #212. Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair. While being used by tourists, every surface of a bathtub, shower, shower enclosure, toilet and lavatory, which may come in contact with a person’s body, must be sanitized each day, unless the guest has declined regular tourist room services. If a tourist declines regular tourist room services, the tourist accommodation facility must ensure that these surfaces are cleaned and sanitized at least once per week and between tourists. 17 - proper ventilation; no sign of mold, mildew, odors or moisture
Regulation: 511-6-2-.08(5) - toilet and bathing ventilation
Observed mold-like accumulation on the mattress in room #113. Toilet, lavatory and bathing facilities shall be mechanically ventilated. Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. In existing tourist accommodations permitted prior to the adoption of this rule, the current ventilation must prevent odor, mold, mildew, and moisture. All new or major renovation of a tourist accommodation will require mechanical ventilation to be installed to the outside of the building separate from heating and air-condition systems. 18 - toilet tissue and soap provided; soap individually wrapped or properly dispensed
Regulation: 511-6-2-.08(9) - toilet tissue available
Observed toilet paper roll stored outside of dispenser in room #219. Toilet tissue shall be provided in a dispenser at each toilet at all times. Corrective Actions: PIC to replace toilet paper dispenser so that toilet paper roll can be stored and dispensed properly
30 - complies with all applicable regulations, permitted, approved operation
Regulation: 511-6-2-.16(1) - swimming pool in compliance with rules
Observed barrier for abandoned pool to be missing metal picket creating an opening greater than 4 inches in diameter. Regulations of the Department and/ county in which govern the design, construction, operation and maintenance of swimming pools or spas shall apply to pools and spas operated in conjunction with a tourist accommodation. Openings in the barrier shall not allow passage of a four inch diameter sphere. If a swimming pool or spa at a bed and breakfast inn cannot comply with applicable regulations, then it must be enclosed with a fence at least four feet in height with a locked gate and used only by family members. In such case, guests shall not be allowed to use swimming pool or spa. Corrective Actions: PIC to repair pool barrier
