6-1A - proper cold holding temperatures

Observed raw chicken(55F) in small reach in cooler holding below 41 degrees F. Foods must be held below 41 degrees F. CA: PIC discarded chicken. Food will not be held in cooler until it is repaired and can hold food at 41 degrees F or below.
18 - insects, rodents, and animals not present
Regulation: 511-6-1.07(5)(k) - controlling pests (pf, c)
Observed live roach in empty rice cooker. Observed live roach in cabinet below rice cookers. Observed no pest control documentation from the past 6 months. CA: PIC will call pest control today to treat for roaches. PIC will keep pest control documents on file for at least one year after treatments.
13A - posted: permit/inspection/choking poster/handwashing
Regulation: 511-6-1.02(1)(f)(1) - permit, displayed (c)
Observed no signed Food Service Permit posted in public view in restaurant. Food Service must be posted in restaurant within customer view. CA: Will send a new copy of Food Service Permit to restaurant to post.
14A - in-use utensils: properly stored
Regulation: 511-6-1.04(4)(k) - in-use utensils, between-use storage (c)
Observed rice scoop stored in container of water below 135 degrees F. Utensils that are stored in water between uses must be in water that is at least 135 degrees F. CA: PIC dumped water and moved scoop to three compartment sink to be washed.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(n) - maintaining premises, free of unnecessary items/litter (c)
Observed broken prep top cooler and three door cooler kept in main kitchen. All equipment that cannot be repaired must be removed from the facility. CA: PIC will have equipment removed.
