Observed onions stored directly next to the mop sink. Observed opened bag of fine ground whole wheat stored in a damanged, large, white container that container cobb webs, dead spider, and debris. Manager advised that the product is no longer in use due to menu changes. Manager was advised to discard both food product and damanged container. Obsevred boxes of wrapped bread stored underneath a leaking pipe in the walk in cooler. Manager removed the box to a dry location in the cooler and was advised to remove the wrapped bread products out of the wet box.

Observed small and large flies in the main kitchen. Observed spiders in the dining area.

Observed walk in cooler pipe leaking.

Observed rice spoon stored in stagnant water of 72F. Manager was advised to keep in-use utensils in hot water of 135F or above or in a clean container with no water to prevent bacteria growth.

Observed 3 wire fryer baskets in disrepair. Manager was advised that the items must be discarded and replaced.

Observed microwave rusted interior and paint peeling. Manager was advised to replace the microwave. Observed caulking of the 3 compartment sink in disrepair. Manager was advised to remove caulking, clean and sanitize the area, and re-caulk the sinks.

Observed ceiling tiles in the main kitchen buckling. Observed ceiling tile missing in the dining. Observed cobwebs on walls in the dining area.

Observed rust and mold-like substance on the air vents in the main kitchen. Observed dust accumulation on the walk-in cooler fan.

Regulation: 511-6-1.07(5)(j) - using dressing rooms & lockers, used for changing clothes and storing employee belongings (c)
Observed employee personal items (apple watch, phones, takis chips, and wallet) stored on dry storage shelves.
