Observed ice build up in the reach in freezer and on the exterior of the freezer due to a condensation leak. PIC has placed a work order. Once the repair is completed, the PIC will email a copy of the completed work order to Robert.Lewis1@dph.ga.gov. The repairs will be completed by 03/15/2026.

Observed the reach in freezer door does not properly seal. PIC will contact maintenance to have the gasket replaced and/or door repaired.

Observed wiping cloth buckets with towels did not reach the minimum required strength of 150-400 PPM of quaternary ammonium. Sanitizer buckets observed at 100 ppm. PIC corrected on site by remaking wiping cloth buckets to 200 ppm of quat sanitizer.

1. Cloths in-use for wiping food spills from tableware and carry-out containers that occur as food is being served shall be:

(i) Maintained dry; and

(ii) Used for no other purpose.

2. Cloths in-use for wiping counters and other equipment surfaces shall be:

(i) Held between uses in a chemical sanitizer solution at a concentration specified under DPH Rule 511-6-1-.05(6)(n); and

(ii) Laundered daily.

3. Cloths in-use for wiping surfaces in contact with raw animal foods shall be kept separate from cloths used for other purposes.

4. Dry wiping cloths and the chemical sanitizing solutions in which wet wiping cloths are held between uses shall be free of food debris and visible soil.

5. Containers of chemical sanitizing solutions in which wet wiping cloths are held between uses shall be stored off the floor and used in a manner that prevents contamination of food, equipment, utensils, linens, single-service, or single-use articles.

6. Single-use disposable sanitizer wipes shall be used in accordance with EPA-approved manufacturer’s label use instructions.

Observed a black mold-like substance on the interior surface of the white chute of the ice machine. Clean the interior surface so that it is clean to the sight and touch.

1. Equipment food-contact surfaces and utensils shall be clean to sight and touch.

Observed a dead insect in the dishwasher. (Repeat)

Observed wet dishes stored on storage racks with holes underneath. The dishes will need to properly air dry on the drain boards then moved for storage on the racks.

Observed a dirty utensil stored with clean dishes. PIC removed to clean (COS).

Observed garbage debris around the outside dumpster.

Observed water nesting on the floor in the dish room and under the dish storage racks. Also dust debris on ceiling tile in the dry storage room.

Observed the AC vent in the dry storage room in need of cleaning (dust debris).

Observed a few light shields throughout the kitchen with debris in them (appears to be bugs).
