511-6-2-.15(5) - Heating and Fire Safety-Inspection All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

Observed the wall in the laundry room in bad repair. PIC will have the laundry room repaired by 06/15/26.

511-6-2-.17(1) - Laundry Facilities Where laundry facilities are provided, they shall be separate from other facilities, of sound construction, and shall be kept clean, and in good repair. Laundry rooms for guest use shall be vented to the exterior and well lighted. The tourist laundry equipment provided for tourists to use shall be separate from the tourist accommodation laundry facility. Laundry equipment shall be provided with hot and cold water under pressure. Dryers shall be vented to the outside. A storage area or room shall be required to store clean linens and laundry at least a minimum of 6-inches off the floor and be free of pests, vermin, dust and moisture. If a tourist accommodation does not have an approved laundry facility, the tourist accommodation owner may contract with an approved laundry service provider. In all newly constructed permitted facilities, and existing facilities remodeled or renovated after the adoption of these rules, except for bed and breakfast inn, a floor drain located to receive intentional or accidental drainage from equipment or plumbing shall be required in laundry facilities

Observed stains on the mattress and hair build up on the mattress cover in room number 3. PIC will replace the mattress cover and clean the mattress by 11/7/25

Observed cleaning bottles in the laundry room that did not have an identifying label. All chemical bottles must be properly labeled with the common name of the chemical

Observed several chemical bottles in the laundry room that were not properly labeled with the common name of the chemical. PIC labeled chemical bottles as a corrective action on site.

511-6-2-.17(2) - Housekeeping Carts Housekeeper carts shall be so arranged that clean replacement supplies, clean linens, and cleaned and sanitized multi-use equipment and utensils shall be protected from soiled items being removed from each room. Soiled linens and refuse shall be placed in appropriate containers on housekeepers’ carts. Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents. Cleaners and sanitizer shall be used according to the manufacturer’s use direction and label

Room 14: Observed staining on towels. Manager removed the towels. Also observed staining and debris in the bottom of shower
