Observed loose screws and debris used to repair the two door cooler in the motor area of the cooler. PIC corrected on site by removing all objects that could lead to physical contamination.

511-6-1.05(7)(a)2,3 - Equipment, Food/Nonfood-Contact Surfaces, and Utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (C) 2. The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations.

3. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Observed quaternary ammonium sanitizer in the three compartment sink above 400 ppm. PIC was advised to add water to reduce the concentration to a range below 200-400 ppm. PIC corrected on site by remaking the sanitizer and sanitizer buckets.

Observed the reach in warmer cabinet door handle in bad repair. The door handle must be repaired to prevent injury. PIC will place a work order to have the door repaired by 10/15/25.

Observed an open bottle of water stored in the reach in cooler. PIC advised to not store open bottles of water in coolers in the food service area unless the cooler is a designated employee cooler.

Observed the ice machine chute with a brown (slime-like) substance on it. The machine will need to be cleaned and the ice removed.

Observed metal pans in the reach in cooler catching condensation from the top of the coolers. The coolers will need to be serviced o see if the condensation lines are clogged up. Units must be maintained in good repair.

Observed the ice machine with black mold substance in contact with ice. (COS). PIC started the cleaning process. Please replace the ice machine chute to reduce mold build-up.

Observed two insect control devices (sticky pads) with excessive insects on them. Also observed spiders in the bathroom along with spiderwebs. Spider webs were also observed under and in front of the ventilation hood system.

Observed the walk-in cooler with dust debris on the ceiling, front facing vent system, and the protective shield on the light.

Observed the standing fan with dust on the facing.

Observed two lights out under the ventilation hood system.

Observed HVAC vent with rust build-up. The vents can be painted or replaced.
