Observed several fruit flies throughout the kitchen (front & back) as well as the lobby area. Pest control will need to treat the facility for fruit flies within the next week by 11/14/25 and again by 11/28/25 (twice for November) to help eliminate the small pest. Observed the ice machine with a thick black mold-like substance on the interior surface and a brown substance on the black interior surface of the machine. Advised the PIC to put a sign on the machine stating "Do not use" until the machine has been properly washed, rinsed, and sanitized. Facility will have to use bags of ice until the machine has had a deep cleaning.

After review, the suctioning lines for the sanitizer and dish detergent are not properly secure to pump the right amount of concentration. Advised PIC to call ECOLAB ASAP to service both lines.
