Observed a egg and cheddar tray with an expiration date of 11/6/25. PIC removed the tray for waste (COS).

511-6-1.04(8)(b) - Expired Foods (P) (b) Expired Foods. Prepackaged sandwiches, eggs, infant formula, shucked oysters, milk, and time/temperature control safety foods that are labeled as “keep refrigerated” and that are for sale or service to the consumer or used as an ingredient in other foods shall be immediately discarded and shall not be sold, served, or used after the manufacturer’s expiration date or the sell-by date. P

Observed several fruit flies throughout the kitchen (front & back) as well as the lobby area. Pest control will need to treat the facility for fruit flies within the next week by 11/14/25 and again by 11/28/25 (twice for November) to help eliminate the small pest. Please email both service reports with findings and recommendations to sabrina.young@dph.ga.gov after each treatment.

511-6-1.07(5)(k) - Controlling Pests (Pf, C) (k) Controlling Pests. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by:

1. Routinely inspecting incoming shipments of food and supplies;

2. Routinely inspecting the premises for evidence of pests;

3. Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule; Pf and

4. Eliminating harborage conditions.

Observed the ice bin on top of the ice machine not inverted to protect from contamination. PIC removed the bin for cleaning and inverted it (COS).

511-6-1.05(10)(e)1,2,4 - Equipment, Utensil, Linens, stored 6" off floor in clean, dry location (C) (e) Equipment, Utensils, Linens, and Single-Service and Single-Use Articles.

1. Except as specified in paragraph 4 of this subsection, cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored:

(i) In a clean, dry location;

(ii) Where they are not exposed to splash, dust, or other contamination; and

(iii) At least 6 inches (15 centimeters) above the floor. 2. Clean equipment and utensils shall be stored as specified under paragraph 1 of this subsection and shall be stored:

(i) In a self-draining position that allows air drying; and

(ii) Covered or inverted. 4. Items that are kept in closed packages may be stored less than 6 inches (15 centimeters) above the floor on dollies, pallets, racks, and skids that are designed as specified under subsection (2)(hh) of this Rule.

Observed the CFSM document posted for "Kyle Johnson" is no longer valid due to Kyle not working for the store anymore. The facility does not have a CFSM. Facility is required to have a certified food manager for the establishment. Certificate must be obtain within 60 days.

Observed the ice machine with a thick black mold-like substance on the interior surface and a brown substance on the black interior surface of the machine. Advised the PIC to put a sign on the machine stating "Do not use" until the machine has been properly washed, rinsed, and sanitized. Facility will have to use bags of ice until the machine has had a deep cleaning.

Observed the small sanitizer bowls on the front-end at 100 PPM quaternary ammonium solution. After review, the suctioning lines for the sanitizer and dish detergent are not properly secure to pump the right amount of concentration. Advised PIC to call ECOLAB ASAP to service both lines. As a corrective action facility will use FDA approved wipes and the dishwasher (COS).

Observed a few reach in coolers on the front-end without thermometers are thermometers that were damaged and not working. All cold holding units are required to have a thermometer located in the warmest part of the unit. Advised PIC to check thermometers and order some for the units that had them missing.

Observed several employees handling and prepping food items without a hair or beard restraint. Advised PIC that hair and beards will need to be restrained to reduce contamination of food products.

Observed a few flies on the front-end and the back of the house. PIC stated that the store has been having issues with flies. ECOLAB treated on 10/10 for flies, but no activity reported. Advised PIC to call ECOLAB and schedule for them to treat the facility for flies both interior and exterior by 11/20/24.

Observed single-services cups stored on the floor near the office. PIC stated that the cups had been stored there for about a week. Advised PIC to make sure all food related items are stored off the floor on delivery day.

Observed the women's restroom without a garbage can with a lid. Advised the PIC that a garbage can with a lid is required for personal items.

Observed an excessive amount of garbage, debris, and wood pallets stored in the enclosed outside dumpster area. This area will need to be cleaned by 11/20/24. Please email pictures showing the area cleaned to sabrina.young@dph.ga.gov by 11/20.

Observed (2) chemical bottles improperly stored on the front and back of the facility. PIC removed the bottles and stored them properly. (COS)

Observed a cellphone laying on the prep top surface. Please store personal items in a designated area. PIC removed the phone. (COS)
