Observed the faucet broken on the mop sink located behind the building. PIC unable to properly clean mops. PIC is currently using the toilet to dispose of mop water. PIC will have the plumbing faucet along with running hot and cold water by 4/25/26.

Observed pizza boxes stored below the handsink area. PIC will relocate pizza boxes to shelf across from the handsink to prevent potential contamination by 4/22/2026.

511-6-1.05(10)(e)1&3 - Single-Service/Single-Use items stored 6" off floor in clean, dry location (C) (e) Equipment, Utensils, Linens, and Single-Service and Single-Use Articles.

1. Except as specified in paragraph 4 of this subsection, cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored:

(i) In a clean, dry location;

(ii) Where they are not exposed to splash, dust, or other contamination; and

(iii) At least 6 inches (15 centimeters) above the floor. 3. Single-service and single-use articles shall be stored as specified under paragraph 1 of this subsection and shall be kept in the original protective package or stored by using other means that afford protection from contamination until used.

Observed a lid missing from one of the shared dumpsters. PIC will contact the landlord to have the dumpster lid replaced by 4/25/26. 511-6-1.06(5)(k),(l) - Storing Refuse, Recyclables & Receptacles; Areas, Enclosures, & Receptacles, Good Repair (C) (l) Areas, Enclosures, and Receptacles, Good Repair. Storage areas, enclosures, and receptacles for refuse, recyclables, and returnables shall be maintained in good repair.

Observed build up on the bottom of the preparation table. The area is being used for storage. PIC will clean on a weekly basis. 511-6-1.05(7)(a)2,3 - Equipment, Food/Nonfood-Contact Surfaces, and Utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (C) 2. The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations.

3. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Observed the stores outside dumpster with a broken lid. Please contact the waste management company and have the lid replaced or replace the dumpster. Please replace by 10/15/25.

Observed build up on ceiling tiles around the vents. Also observed build up on floors below the deep fryers.

Observed the back door not properly sealed. Observed sunlight when the door is closed. The door must be properly fitted and sealed.

Observed a pipe leaking inside of the walk in cooler. The water was being caught in a metal pan. Advised PIC to remove all food from underneath the leak to protect it from contamination. The pipe must be fixed by 4/15/25. Please send a picture of the work order to sabrina.young@dph.ga.gov

Observed single service containers and lids stored not inverted to protect from contamination. Advised PIC to make sure clean dishes or single service trays, etc. are stored inverted.

Observed an accumulation of a black mold-like substance on the interior surface of the handwashing sink (front of kitchen).

Observed the splash guard behind the 3-compartment sink in need of cleaning due to an accumulation of debris.

Observed the toilet seat with old urine debris. Please clean regularly.

Observed the store's outside dumpster with a broken lid. Please contact the waste management company and have the lid replaced or replace the dumpster. Please replace by 4/15/25. Please send a picture to sabrina.young@dph.ga.gov once the dumpster has been replaced.

Observed the floors underneath the fryers in need of cleaning due to an accumulation of oil and debris.

Observed the vents on the AC unit in the kitchen in need of cleaning due to build-up of dust.

Observed several food items including the alfredo and cheddar sauce exceeded the expiration date. The sauces were dated 09/27 and 10/10/24. CA- PIC discarded expired product. Rice was not properly dated marked in the reach in cooler. Rice was labeled as a corrective action. PIC will monitor all TCS foods and discard within 7 days.

Observed a missing dumpster lid on one of the dumpsters. Back door not properly sealed. Observed sunlight when the door is closed. The door must be sealed. A weather strip is also need at the bottom of the door. The weather strip will be added by 10/25/24. Self-closure must be added to the backdoor. The PIC advised that she is replacing the door within 30 days.

Observed one of the dumpsters is missing both lids. CA- PIC will contact the landlord to have the dumpster replaced. Dumpster will be replaced by 10/25/24
