Room 134- Observed stained and rusted lamp fixtures. Draw was in disrepair Room 132- Observed build up in the microwave. Draw was in disrepair. Desk chair was in bad repair. Lamp shade damaged and stained Room 101- Reach in cooler shelves rusted. Black mold observed at the top of the lamp fixture. Lamp shade observed in disrepair. Room 207- Observed stained lamp shades and rusted lamp fixtures. Room 223- Observe lamp shade missing. Back of chair in disrepair. Room 242- Headboard contained tape. Lamp shade needs to be replaced. Desk chair contains debris. Furniture in general was in disrepair.
511-6-2-.14(5) - Condition of Furnishings All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin

Room 134- Observed stains on linen Room 132- Hair and stains on linens and mattress cover. Room 101- Stained observed on pillow. Mattress cover was ripped. Mattress was heavily stained Room 207- Observed hole in pillow case and top sheets. Heavy stain observed on mattress cover. Room 223- Observed stains on top sheets and linens. Holes observed in mattress cover. Large stains observed on mattress cover. Room 242- Stains observed on fitted sheets. Hole observed on mattress pad. 511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly

Room 132- Observed mold around caulking of the bath tub. Room 101- Observed stains and hair on towels. Mold on caulking and shower head. Faucet head plate around shower was loose. Room 207- Observed the shower head loose and water leaking behind the faucet. The water pressure was also low in the sink. The room cannot be rented until repairs are completed. Room 223- Observed hair on sink, counter, and tub. Observed stain on walls, hair and stains on towels, paint peeling in the tub, cigarette burns in the tub and mold observed on top of the shower. The counter was also in disrepair. Room 242- Hair and stains observed on shower curtain and towels. Shower rail in disrepair, and open soap observed at the top of the shower

Regulation: 511-6-2-.17(1) - laundry facilities
Holes observed in the laundry room ceiling. Hair observed on clean linens. There was also a chemical bottle that was incorrectly labeled.
511-6-2-.17(1) - Laundry Facilities Where laundry facilities are provided, they shall be separate from other facilities, of sound construction, and shall be kept clean, and in good repair. Laundry rooms for guest use shall be vented to the exterior and well lighted. The tourist laundry equipment provided for tourists to use shall be separate from the tourist accommodation laundry facility. Laundry equipment shall be provided with hot and cold water under pressure. Dryers shall be vented to the outside. A storage area or room shall be required to store clean linens and laundry at least a minimum of 6-inches off the floor and be free of pests, vermin, dust and moisture. If a tourist accommodation does not have an approved laundry facility, the tourist accommodation owner may contract with an approved laundry service provider. In all newly constructed permitted facilities, and existing facilities remodeled or renovated after the adoption of these rules, except for bed and breakfast inn, a floor drain located to receive intentional or accidental drainage from equipment or plumbing shall be required in laundry facilities
