Observed no handwashing sign on the handwashing sink at the bar. The sink must be properly fixed and a handwashing sign must be added immediately. A employee must wash hands sign must be added to all handsinks including the men's restroom.

A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees and shall be clearly visible to food employees. No handwashing sign provided at a handsink used by food employees.

Observed a splash guard needed between the handsink and three compartment sink drainboard in the bar area. PIC will have the splash guard installed by 6/1/26.
511-6-1.05(1)(i) - Nonfood-contact Surfaces (C) (i) Nonfood-contact Surfaces. Nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material.
15B - warewashing facilities: installed, maintained, used; test strips

Observed quaternary ammonium test strips provided by the PIC expired 4/15/2026. PIC discarded and replaced the test strips.

Observed build up on shelves in the cabinet. Also observed build up on the bottom of the preparation table above the area where mixing bowls are stored in the kitchen area. PIC will clean the area by 5/28/26.
17B - garbage/refuse properly disposed; facilities maintained

Observed a lot of trash on the outside behind the dumpster on the dumpster pad. This area must be cleaned and maintained to reduce rodent and bug activity by 5/28/26.

(p) Maintaining Refuse Areas and Enclosures. A storage area and enclosure for refuse, recyclables, or returnables shall be maintained free of unnecessary items and clean.

Observed syrup build up beneath the drink box storage area resulting from a leak. PIC will have the drink lines inspected to determine the source of the leak.

The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.

Except for cleaning that is necessary due to a spill or other accident, cleaning shall be done during periods when the least amount of food is exposed, such as after closing.

If present, playground equipment and associated areas shall be maintained in a clean and sanitary condition. In addition, a cleaning and sanitizing plan along with cleaning and sanitizing records for playground equipment and associated areas shall be maintained within the food service establishment for review upon request by the Health Authority. Further, such cleaning and sanitizing plan for employees to follow in the event of encountering vomitus or fecal matter shall be included and submitted at the time of permit application as specified in DPH Rule 511-6-1-.02(1)(c). Observed build up on the kitchen vent and women's restroom vent.PIC will have both vents cleaned by 5/28/26 to prevent odors and potential contamination of food in the kitchen area.
