Observed no paper towels at the handsink in front food service area. Each handwashing sink or group of adjacent handwashing sinks shall be provided with: Individual, disposable towels. Corrected on Site (COS): PIC put paper towels inside the dispenser.

Observed an employee's Coke zero on shelf next to single-use items for customers. Lockers or other suitable facilities shall be located in a designated room or area where contamination of food, equipment, utensils, linens and single-service and single-use articles cannot occur. Place employee items away from items used for customers.

Observed several food containers that have been wash, rinsed, and sanitized with old food labels on them. Equipment food-contact surfaces and utensils shall be clean to sight and touch. Rewash, rinse, and sanitize the food containers.

Observed 3 working chemical spray bottles without a label for its contents. Working containers used for storing poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material. Place a label on all chemical spray bottles.

Observed an employee actively prepping pizza while wearing a bracelet. Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food. Remove the bracelet.

Observed employees single-use cups with a lid and straw stored on the shelf next to items used for customers. Lockers or other suitable facilities shall be used for the orderly storage of employee clothing and other possessions. Relocate employee drinks away from items used for customers.

Observed salad spinner lid in the handsink. A handwashing sink shall be maintained so that it is accessible at all times for employee use. Person in Charge (PIC) removed item from the handsink.
