Observed refried beans being held above 41F inside the walk in cooler. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F. The Person in charge (PIC) states the food item was prepared the previous night. COS: Food item was discarded. Coach employees on proper cooling methods.

Observed the dish machine expelling over 200ppm of chlorine during a wash cycle. Chlorine sanitization concentration should be from 50-100ppm per manufacturer's instructions. PIC called maintenance and they are on the way to fix the machine. An informal visit will be had to verify the machine has been fixed. Until then, the establishment will be using their three compartment sink for wash, rinse, and sanitizing of dishes and utensils.

Observed a pan of black beans with a discard date of 2/6 and a pan of enchilada sauce with a discard date of 2/5. A food that requires datemarking shall be discarded if it exceeds 7 days with the day of preparation being day 1. Person in charge (PIC) discarded items.

Observed a large container of sugar and black beans stored in the dry storage area without being covered. Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination. Keep food in the dry storage area covered to prevent contamination.

Observed the back door open when not being used. Outer openings of a food service establishment shall be protected against the entry of insects and rodents by solid and tight-fitting doors. Please keep the doors closed when not being used.
