Person in charge could not provide that all employees are aware of foodborne illness transmissible through food. Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food. Provided person in charge with employee health document, so all employees are informed.
2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.07(3)(b) - hand drying provision (pf)
Observed no paper towel at the men's restroom hand sink and the rear hand washing sink in the main kitchen. Each handwashing sink or group of adjacent handwashing sinks shall be provided with: Individual, disposable towels. Person in charge refilled the paper towels.

Observed the front hand washing sink inoperable. A handwashing sink shall be maintained so that it is accessible at all times for employee use. Have the hand sink repaired.

Observed no food allergens posted. The permit holder shall notify consumers by written notification of the presence of major food allergens as an ingredient in unpackaged food items that are served or sold to the consumer. Provided person in charge with a food allergen template to post.
