Observed a leftover bar of soap at the bathroom sink. 511-6-2-.08(4) - Toilet Facilities-Cleaning and Maintenance Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Discard old/used bar soap from bathroom sink

Observed a burnt out lightbulb in the lamp between the two beds in room 24. 511-6-2-.14(4) - Room Lighting All rooms shall be well lighted. When natural light fails to provide sufficient illumination, evenly distributed artificial light shall be provided to maintain a lighting intensity of not less than ten foot candles at 30" above the floor. Replace burnt out lightbulb with new lightbulb

Observed a hole in the sheets in rooms 23 and 30. 511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Replace sheets with burn holes with fresh new clean linens

Observed debris inside the tub in room 24. 511-6-2-.08(4) - Toilet Facilities-Cleaning and Maintenance Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Clean inside the tub as often as necessary

Observed debris inside the microwave and inside the dresser drawer in room 30. 511-6-2-.14(5) - Condition of Furnishings All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin. Clean out the microwave and dresser drawers as often as necessary

Observed debris inside the sink in room 30. Also, observed a dirty toilet in room 30. 511-6-2-.08(4) - Toilet Facilities-Cleaning and Maintenance Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Clean sink and toilet as often as necessary

Observed debris under the bed in room 23 and 30. Also, observed debris on the floor in room 30. 511-6-2-.14(1) - Floors, Walls, and Ceilings Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. Clean under the bed and floor as often as necessary

Observed two live spiders in room 23. 511-6-2-.13(1) - Insect and Rodent Control Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. Have a licensed pest control applicator spray the facility as often as necessary

Clean under the bed as often as necessary
