6-1A - proper cold holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; cold holding (p)
Observed multiple time/temperature control for safety (TCS) items stored on counter not under refrigeration, above 41F. Corrective Action (CA): Person-in Charge (PIC) instructed to ensure all time/temperature control for safety foods are stored at the proper temperature of 41F or below. PIC was instructed to use time as a public health control on items, items labeled with discard time. COS.
11A - proper cooling methods used: adequate equipment for temperature control
Regulation: 511-6-1.05(3)(a) - cooling, heating, and holding capacities (pf)
No make table/prep top unit at establishment causing TCS items to be stored on counter, insufficient cold holding capabilities. PIC instructed to obtain commercial cold holding unit for all TCS items.

Observed corn dogs thawing on counter, not thawing under refrigeration or under cold running water.
14B - utensils, equipment and linens: properly stored, dried, handled

Observed clean food pans not stored inverted on back clean dish shelf.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(d) - nonfood-contact surfaces (c)
Observed inside of reach-in cooler dirty, ensure all not food contact surfaces are maintained.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed dirty return air vent on ceiling of kitchen.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
Observed employee items (phones, jackets, purses) stored above single service items in back storage room. Improperly located employee storage location. REPEAT.
