Observed quaternary ammonia sanitizer concentration in three compartment sink below 200ppm. Corrective Action (CA): Person in Charge (PIC) instructed to ensure quaternary ammonia sanitizers remain between 200-400ppm.

Observed damaged ceiling tiles near back door of establishment. Observed accumulations on ceiling tiles above pizza oven. REPEAT.
