Regulation: 511-6-1.03(1)(c)17 - demonstration (pf)
Manager/staff could not demonstrate knowledge of diseases/symptoms that required reporting and could not demonstrate knowledge of exclusion/restriction protocol for ill employees.
2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.06(2)(o) - using a handwashing sink- operation & maintenance (pf)
Observed the hand wash sink not accessible during inspection. Observed blenders in front of hand sink. Manager corrected violation by moving blenders to another location.
2-2E - response procedures for vomiting & diarrheal events
Regulation: 511-6-1.03(6) - clean-up of vomiting and diarrheal events(pf)
Manager/staff was unaware and could not provide cleanup procedures for vomit/diarrheal events for establishment. Management was given information on cleanup procedures and plans to adopt them into standard procedures for the establishment.
18 - insects, rodents, and animals not present

Observed numerous flies in establishment and air curtains at doors not operating at time of inspection.

Regulation: 511-6-1.05(10)(e)1&3 - single-service/single-use items stored 6" off floor in clean, dry location (c)
Observed boxes of single service items stored in contact with floor in dry storage area.
