6-1A - proper cold holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; cold holding (p)
Observed TCS foods (deli meats, cheese, sliced tomatoes) stored on prep top sandwich cooler above 41F. Corrected on site by discarding TCS items and placing ice in cooler. Facility will need to use ice until the unit is repaired and confirmed to be holding the TCS items at 41F or below.
Environmental Health will follow-up on 4/17.
2-2E - response procedures for vomiting & diarrheal events
Regulation: 511-6-1.03(6) - clean-up of vomiting and diarrheal events(pf)
Facility could not show that they had a disinfectant on site that is effective against norovirus. Facility provided a written procedure using chlorine bleach as a disinfectant that is not available in the facility.
A food establishment shall have written procedures for employees to follow and an EPA registered disinfectant effective against Norovirus available on-site at the food service establishment when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food service establishment. The procedures shall address the specific actions employees must take to minimize the spread of contamination and the exposure of employees, consumers, food, and surfaces to vomitus or fecal matter.Pf

Facility could not provide water testing results for quarterly bacteria analysis during course of inspection.
The most recent sample report for the non-public water system shall be retained on file in the food service establishment or the report shall be maintained as specified by the Department.

The facility could not demonstrate that there is a written notification of the presence of major food allergens in the facility.

The permit holder shall notify consumers by written notification of the presence of major food allergens as an ingredient in unpackaged food items that are served or sold to the consumer.

Observed pitted ceiling tile in rear kitchen area. This must be replaced with a smooth, nonporous tile.
