Room 317 observed with tear on right bed sheet, pillow case has yellow stains, left bed pillow with red & brown stains, mattress protector with stains, and mattress badly brown staining. Room 223 with mattress staining on sides and bedding cover & skirt with stains. Bed insert observed with lots of holes & burns, few stains. Mattress observed with tear underneath

Ice machine chutes and drip pans (1st, 2nd, & 3rd Floor) observed with black mold. Must be cleaned, send verification to Bernotta.Linder@dph.ga.gov

Several mattress protectors, bed skirt, & bed insert observed badly stained. Room 418 bed insert with yellow staining, room 327 mattress protector stained and top mattress along seam (bed closer to window), and room 119 mattress protector & bed skirt with stains

Stains observed on cushion of upholstery sofa chairs in rooms 407, 314, & 106. Side of upholstery sofa chair in room 407 observed torn along side

Stains observed on white sheer curtain in room 411, room 314 sheer curtain had small discolored stain, and room 106 with stains on short grey black-out curtain, large stains on long floor-length curtain
